Thus, healthcareprofessionals focus on assessing the setting of the environment when patent care will be delivered prior to the treatment in order to ensure better outcomes (Fawaz et al. 2018). The assessment will thus focus on assessing different factors of health care environment based on the case studyof a 35 years old Joseph who is paralyzed on his left side and thus need home care after discharge from the hospital.

It has a significant role in developing an appropriate and well-structured understanding of the needs and issues of an individual and providing assessments such as in-person visits, self-assessment and others according to their demands (Bywater et al. 2018). Thus, it will help in providing Joseph and his family with the opportunity to seek better assessment were their issues, demands and need will be assessed and supportive measures will be provided. Apart from health assessmentand fundings, social care support and home-basedhealth care assistance should be provided to Josephbasedon his health needs and issues.

Community Inclusion & Participation
Involvement of community and participation in the community has a significant role in not only enhancing individual health and well being but also helping develop community enhancement. Community connectedness helps in developing a healing and recoveryprocess where people belonging to the community serves in a collaborative process working together to help each other within the society. Focusing on the case study of Joseph it has been found that he has been an active member of the community and had a friend who lives locally thus, it helped him sustainsupport in a huge number from the people within the community (Griffin et al. 2020). He has been sociallyactivewithin the community and thus had a better connection with the families living in the neighbourhood. In the case of Joseph, it is necessary to providejosephempowermentand independence to lead his life and daily activities without assistance as it will help him sustainbetter recovery. In order to do so professional needs to help him in the initial stage, motivate him as well as guide him through recovery options.
